How to Begin Supporting the Maines
- Decide whether you want to donate via personal check, a monthly credit/debit payment, or online.
- To give securely online click here.
- Download, print, and fill out the Missionary Faith Promise Form
- If giving by credit or debit card, but not giving online, you must also download, print and fill out the Credit Card authorization form.
- If giving by check, write your first check and make it out to “Assemblies of God US Missions” and include “Mark Maine #2591873″ on the memo line of the check.
- Make sure all forms and checks are filled out completely and mail them to the following address: US Missions, 1445 N. Boonville Ave., Springfield, MO 65802
